PDGF vs. Under-Eye Filler: Which Is Better for Under-Eye Rejuvenation?
If your under eyes are starting to look tired, hollow, crepey, or simply different than they used to, you may be wondering whether under-eye filler is still the best option.
For years, hyaluronic acid filler was one of the most common treatments for under-eye hollowness. Today, regenerative treatments like PDGF are giving patients another option, particularly for those who want to improve the under-eye area without adding significant volume.
So, what’s the difference between PDGF and under-eye filler, and how do you know which one is right for you?

What Is Under-Eye Filler?
Under-eye filler typically uses a hyaluronic acid dermal filler to restore volume in the tear trough or under-eye area.
For the right patient, carefully placed filler can soften the transition between the lower eyelid and cheek and improve the appearance of true volume loss.
But the under-eye area is anatomically complex, and not every shadow or hollow is caused by a lack of volume.
In some patients, adding filler can contribute to prolonged swelling, puffiness, or an unnatural appearance. This is one reason thoughtful patient selection is especially important when treating beneath the eyes.
What Is PDGF for the Under Eyes?
PDGF stands for platelet-derived growth factor.
Rather than functioning like a traditional dermal filler, PDGF is used as a regenerative treatment to support cellular repair and the body’s natural tissue-renewal processes.
When used beneath the eyes, the goal isn’t simply to fill a hollow. Instead, PDGF can be used to support improvements in skin quality, texture, and the overall appearance of delicate under-eye tissue.
For patients who aren’t looking for additional volume, this can make PDGF an appealing option for under-eye rejuvenation.
PDGF vs. Under-Eye Filler: What’s the Difference?
The biggest difference is what we’re trying to accomplish.
Under-eye filler primarily addresses volume.
PDGF primarily takes a regenerative approach to tissue and skin quality.
If someone has a true structural hollow and otherwise healthy under-eye tissue, a carefully selected filler treatment may be appropriate.
But if the primary concern is thin or crepey skin, fine lines, textural changes, or an overall tired appearance, simply adding volume may not address the underlying concern.
That’s where regenerative treatments become especially interesting.
Why I Don’t Automatically Reach for Under-Eye Filler
One of the most important parts of treating the under-eye area is recognizing that a hollow doesn’t always need to be filled.
The appearance of dark circles or hollowness can come from several factors, including facial anatomy, skin thickness, pigmentation, volume loss, vascular show-through, and the relationship between the lower eyelid and cheek.
Treating every under-eye concern with filler can sometimes create more fullness without actually improving what was bothering the patient in the first place.
My approach is to first identify why the area looks tired.
From there, we can decide whether the tissue needs support, the skin quality needs improvement, structural volume restoration is appropriate, or whether treatment isn’t necessary at all.
Who May Be a Good Candidate for Under-Eye PDGF?
PDGF may be worth considering if your concerns include:
Crepey or thin-looking under-eye skin
Fine lines
Changes in skin texture
Mild hollowness
A tired or less refreshed appearance
A preference for regenerative treatments
A desire to avoid adding significant volume beneath the eyes
The right treatment still depends on your anatomy and the underlying cause of your concern.
Is PDGF Better Than PRF for Under Eyes?
PRF and PDGF are both used within regenerative aesthetics, but they aren’t exactly the same.
PRF is created from a patient’s own blood and contains platelets, fibrin, and naturally occurring growth factors. PDGF treatments use concentrated platelet-derived growth factor to provide regenerative signaling without requiring a blood draw.
Neither treatment is universally “better.”
The best option depends on your skin, anatomy, goals, treatment plan, and your provider’s assessment.
Does PDGF Replace Under-Eye Filler?
Not necessarily.
PDGF and filler are different tools.
There are patients who may benefit from structural volume restoration with filler, patients who are better suited for regenerative treatments, and patients who may benefit from a combination approach.
The goal shouldn’t be to choose the newest treatment.
It should be to choose the treatment that makes the most sense for your face.
A Preservation-Focused Approach to Under-Eye Rejuvenation
